Terms and Conditions
I hereby make application to enroll in Dentex Dental Plan, Inc. for a minimum of 1 year. I hold Dentex blameless for any negligence on the part of the participating provider and agree to discuss all fees with the provider before I receive treatment. Dentex may terminate this agreement without cause by sending a notice of termination to the email address provided.
I understand Dentex is a discount dental plan and not a dental insurance company.
Payment Conditions:
Monthly Memberships:
Credit Card or Automatic Bank Drafts: I authorize Dentex to deduct my monthly membership fee on the 3rd business day of each month for a minimum of 1 year. Dentex will continue drafting until notified of cancellation in writing. Cancellation notice must be submitted 30 days prior to draft and may be emailed to admin@dentex.net. No refunds.
Yearly Memberships:
One-Time Annual Payment: I understand that my membership will expire 1 year from my enrollment date. My plan will not be automatically renewed. Dentex will notify me when my expiration date is approaching. My account will become inactive if not renewed by the due date and letting the plan lapse will result in a $25 enrollment fee if I decide to join again. No refunds.
Automatic Renewal: I agree to enroll in the Auto-Renew program. I understand that by choosing this option my membership will automatically renew and my credit card on file will be charged for my selected plan every year until I cancel. To avoid being charged, I must unsubscribe at least 1 day before the renewal date by managing my online account and opting out of the auto renew option. No refunds.
Satisfaction Guaranteed
If you are unsatisfied with the plan, you may cancel within the first 30 days of enrollment and receive a refund less the $25 enrollment fee. Cancellation requests must be in writing and may be submitted by email to admin@dentex.net.
No refunds will be given after the plan is successfully used for a discounted service.
